Please include only the information needed for the clinic to respond to your enquiry. Do not use the form for emergencies or highly sensitive medical records.

Your information is used to respond to your enquiry, consider whether an appointment is appropriate, arrange or change an appointment and communicate with you about the requested service.
The clinic relies on steps taken at your request before providing a service, or performance of a service agreement, for ordinary contact information. Where you voluntarily provide health information through the form, the clinic relies on your explicit consent to process that information for the purpose of responding to your enquiry. The website is hosted by Netlify. Appointment requests submitted through the website may be processed and stored using Netlify Forms and delivered to the clinic’s email service. These providers act as service providers to the clinic and may process data outside the United Kingdom subject to their applicable data-protection safeguards.
Your information is not sold. It is disclosed only where needed to provide the requested service, operate the website securely, comply with a legal obligation or protect vital interests.
How long enquiries are kept
Appointment-enquiry information submitted through the website is normally retained for 12 months from the last relevant communication and then securely deleted, unless it becomes part of a clinical record or must be retained for a longer period for legal, regulatory or safeguarding reasons. Clinical records are managed separately and may have different retention requirements.
Your rights
Depending on the circumstances, you may have rights to request access to your personal information, correction, deletion, restriction or objection, and to receive certain information in a portable format. You may also withdraw consent where consent is the basis for processing.
